Psychosocial rehabilitation program offering both inpatient and outpatient services for individuals diagnosed with SPMI (Severe and Persistent Mental Illness).

- Case management for clients who meet SPMI criteria
- Psychiatric services and medication management
- Individual therapy and day treatment groups
- Transitional residential (group home) options
- Outpatient therapy & case management
PROGRAM PERKS
- Drop-In Center: Mon–Thurs, 9 AM–3 PM
- Social activities, groups, skill-building
- ADL (daily living) skills: hygiene, cooking, cleaning, budgeting
- Lunch provided for group attendees Mon–Thurs
CASE MANAGEMENT SUPPORT INCLUDES
- SSI/SSDI applications
- Budget building
- Housing support
- Transportation coordination
- Referrals to additional community services
